EDとは

正常な勃起のためにはペニスの中の海綿体という血管の塊に血液が十分に充満し、維持できることが必要です。しかし、さまざまな要因によって、常な勃起をする事が出なきなってしまう事を「ＥＤ（Erectile Dysfunction）（勃起不全、勃起障害）」といいます。

EDの分類と原因

EDは、血管や神経の障害による器質的EDと精神的ストレスなどによる機能的ED（心因性ED）に大きく分類されます。血管の障害原因として加齢、高血圧、喫煙、糖尿病、脂質異常などがあげられます。神経の障害原因としては糖尿病、骨盤内手術などです。機能的（心因性）の原因としてはうつ病などの精神疾患があげられます。

治療法

EDの診断を行い、原因を特定します。問診で症状を聞くだけではなく、心理的な問題がないか等含め具体的にお伺いします。

器質性の可能性も考え、血液検査や画像検査なども必要に応じて行います。治療では性器の血管を拡張させる作用のある治療薬を用い、勃起や勃起の維持をサポートします。

ただし心血管系に障害のある場合や肝機能障害がある場合、内服できないこともあるので詳しくは医師にご相談ください。

2022年4月の不妊治療保険診療化からED治療薬が保険適応に

不妊治療の場合のみED治療薬が保険適応になります。避妊治療以外日本で使えるED治療薬は3種類あり、バイアグラ（シルデナフィル）、レビトラ（バルデナフィル）、シアリス（タダラフィル）です。先発品に加え各種ジェネリックもあります。

ED

For a proper erection, it is necessary for the blood vessels in the penis, specifically in the spongy tissues called the corpora cavernosa, to be sufficiently filled with blood and maintained. However, various factors can prevent the achievement and maintenance of a consistent erection, and this condition is referred to as "ED" or "Erectile Dysfunction," which signifies a difficulty or impairment in achieving a complete and sustained erection.

Classification and Causes of Erectile Dysfunction (ED)

ED is broadly classified into organic ED, caused by vascular and neurological disorders, and functional ED (psychogenic ED), resulting from psychological stress and other non-organic factors. Vascular factors contributing to ED include aging, hypertension, smoking, diabetes, and lipid abnormalities. Neurological factors involve conditions like diabetes and pelvic surgeries. Psychogenic factors, such as depression, contribute to functional ED.

Ttreatment method

The diagnosis of ED involves identifying the underlying causes. This process goes beyond simply asking about symptoms during the medical history; it also includes a detailed inquiry into potential psychological issues and other relevant factors.

Considering the possibility of organic causes, we may conduct blood tests or imaging studies as needed. In treatment, medications with vasodilatory effects on genital blood vessels are used to support achieving and maintaining an erection.

However, it may not be suitable for internal use in cases of cardiovascular system disorders or liver function impairment, so please consult with your doctor for more details.

Starting from April 2022, with the inclusion of infertility treatment in insurance coverage, ED (erectile dysfunction) medications have become eligible for insurance coverage.

ED medications are eligible for insurance coverage only in the case of infertility treatment. In addition to contraception treatment, there are three types of ED medications available in Japan: Viagra (Sildenafil), Levitra (Vardenafil), and Cialis (Tadalafil). Besides the brand-name versions, various generic options are also available.
